Mission Statement

The Sussex Warren County Regional Chronic Disease Coalition is committed to maintaining a diverse partnership of community members and agencies. This Coalition will be dedicated to the implementation of evidence/practice based policy, environmental, and system change intervention for chronic disease prevention and control with a focus on cancer.

Chronic Disease Grant Goals and Objectives

Overarching Goal for 2016-2017

During the 2016-2017 grant period, the Sussex Warren Chronic Disease Coalition will focus on the collaboration with partners to enhance the capacity for development and delivery of projects that impact Policy, Environmental and System Change Strategies (PES) throughout the region. An educational component will be created for each project to bridge the gap between the community stakeholders' knowledge and PES Strategies.

1. Tobacco

GOAL #1: Decrease second hand smoke exposure within the region to improve the health quality of the region's population.

Objective #1a:

The Coalition will work with the New Jersey Prevention Network (NJPN) grantee to increase smoke free parks and recreation areas in two municipalities per county by May 30, 2017.

Objective #1b:

By May 30, 2017, municipal leaders in two selected municipalities per county will receive an educational program by the OTC grantee coordinator to increase awareness about second hand smoke and smoke free parks and recreation areas with assistance of the OTC Coordinator.

Objective #1c:

The Coalition will work with NJPAC, American Lung Association/Pediatric Adult Asthma Coalition of NJ to institute an evidence-based asthma program in one school within the region by May 30, 2017.

Objective #1d:

Provide one Ask, Advise and Refer training through Mom's Quit Connection to one organization within the region by May 30, 2017.

Objective #1e:

Conduct 2 educational and awareness activities on tobacco cessation with priority target groups by May 2017.

Objective #1f:

Schedule one tobacco use related awareness and educational activity each quarter for the OTC grantee to deliver to middle and high school students.

Objective #1g:

Utilize the smoke free housing toolkit to provide technical assistance to 3 housing authorities, low income housing units, or multi-housing in low socioeconomic communities by June 2017.

2. HPV Initiative

GOAL #2 HPV/Cervical Cancer: Educate health care providers about adolescent recommendations for HPV vaccine to increase the HPV vaccination rate.

Objective #2a:

Collaborate with the four practices enrolled in the National Immunization Partnership with the Academic Pediatric Association to increase immunization completion rates, adopting office policy changes and implementing patient centered approaches by June 30, 2017.

Objective #2b:

Provide updated HPV and Cervical Cancer educational materials to school nurses within the region about the importance of HPV vaccination and immunization completion rates by May 30, 2017.

Objective #2c:

Maintain a contact list of the health care providers that received outreach and training to coordinate follow-up activities by June 30, 2017.

Objective #2d:

Collaborate with MERCK to create a toolkit available to providers that will increase HPV vaccination completion rates in the region by June 30, 2017.

Objective #2e:

Collaborate with the Office of Cancer Control, Horizon Blue Cross and Blue Shield to update the HPV manual by June 30, 2017.

Objective #2f:

Conduct one activity for Cervical Cancer Awareness month with NJCEED during January 2017.

Objective #2g:

Conduct one activity for Ovarian Cancer Awareness month with NJCEED in September 2016.

3. Colorectal Toolkit

Goal #3: To achieve a health system change by the implementation of colorectal cancer screenings in physician practices improve their screening performance.

Objective #3a:

Add one or more physician practices, FQHCs or NJCEED programs that agree to implement the American Cancer Society's Colorectal Cancer Screening Toolkit within the region by February 28, 2017.

Objective #3b:

Provide educational presentations to physician practices, FQHCs, or NJCEED programs who agree to implement the American Cancer Society's Colorectal Toolkit within the region by February 28, 2017.

Objective #3c:

Provide support services for the collection of colorectal screening data to all participating physician practices, FQHCs, or NJCEED programs in the region by May 30, 2017.

Objective #3d

Conduct one activity for Colorectal Cancer Awareness month with NJCEED during March 2017.

4. Media

Goal # 4: Use health communication strategies and health information technologies to improve population health outcomes and health care quality.

Objective #4a:

Maintain the Media and Communication Workgroup to utilize health communication strategies to deliver accurate, accessible, and actionable health information each quarter of the grant cycle.

Objective #4b:

Publish one print media communication (e.g. newsletter, article or press release) each month during the 2016-2017 grant cycle.

Objective #4c:

One per month, publish one or more messages, posts or tweets on social support networks to promote a connection with culturally diverse and hard-to-reach populations during the 2016-2017 grant cycle.

Objective #4d:

During the month of September 2016 and June 2017, publish one print media communication and weekly messages, posts and/or tweets on social support networks about prostate cancer in collaboration with NJCEED.

5. Children and Adolescents Health Initiative

GOAL #5: Prevent the increase in obesity within the child and adolescent population that enhance the overall health through the promotion of policy, environmental and systems change (PES) strategy in Sussex and Warren County aligned with Healthy NJ 2020.

Objective #5a:

Implement a healthy eating policy, environmental and systems change intervention for school-aged population (up to age 19) in two or more schools within the region by May 30, 2017.

Objective #5b:

Implement a physical activity policy, environmental and systems change intervention for school-aged population (up to age 19) in two or more schools within the region by May 30, 2017.

Objective #5c:

The Coalition will work with CHIP Coordinators to plan and conduct one or more physical activity programs for children within the region by May 30, 2017.

Objective #5d:

The Coalition will work with NJ SNAP-Ed to plan and conduct one nutrition activity in each county during March 2017.

6. Obesity, Cardiovascular and Diabetes

GOAL #6: Prevent the increase in obesity within the adult population through the promotion of PES in Sussex and Warren County aligned with Healthy NJ 2020.

Objective #6a:

The Coalition will work with the Office of Aging to recruit adults suffering from chronic disease to participate in four Chronic Disease Self-Management and/or Diabetes Self-Management workshops by May 30, 2017.

Objective #6b:

Contribute to annual farmer's markets within the region to increase access to fresh fruits and vegetables for low income populations by May 30, 2017.

Objective #6c:

Increase farmers market voucher acceptance throughout the region in collaborating with NJ SNAP-Ed by June 30, 2017.

Objective #6d:

The Coalition will work with NJCEED and the American Heart Association to conduct one activity for Heart Health Awareness Month during February 2017.

7. Melanoma-Choose Your Cover and SCREEN

GOAL #7: Sun Safety: Increase sun safe behaviors among the region's population and increase early detection of melanoma and other skin cancers.

Objective #7a:

One Choose your Cover free skin cancer screening event will be conducted within each county by June 30, 2016.

Objective #7b:

Within each county, recruit one additional school, pool, beach or camp to implement the evidence-based Skin Cancer Reduction and Early Education Network program (SCREEN) by June 30, 2017.

Objective 7c:

The Coalition will work with the Enright Foundation to promote the Sun Safe Certification program within one or more library or school by June 30, 2017.

8. Breast Health

GOAL #8: Increase breast health awareness and early detection behaviors among the region's population to decrease late state breast cancer diagnosis.

Objective #8a:

By May 30, 2017, present two or more health care providers with educational information to increase awareness of community-clinical linkages that provide access to free or reduced cost mammograms within the region.

Objective #8b:

Conduct one activity for Breast Health Awareness month with NJCEED during October 2016.

Objective #8c:

Conduct one activity for Breast Cancer Survivorship Awareness month with NJCEED during May 2017.

9. Coalition Building and Collaboration

Goal #9: Maintain existing Regional Chronic Disease Coalition members and recruit additional chronic disease stakeholders to work jointly to implement evidenced/practice -based policy, environmental and system change interventions to reduce burden of chronic disease.

Objective #9a:

Two part-time Sussex Warren County Coordinator positions will be maintained throughout the 2016-2017 grant cycle.

Objective #9b:

Hold four Sussex Warren Chronic Disease Coalition quarterly meetings during the grant 2016-2017 cycle.

Objective #9c:

Sussex Warren Chronic Disease Coalition will review and update the mission statement during the first quarterly meeting in August 2016.

Objective #9d:

A Stakeholders Assessment and Gap Analysis will be conducted at the first quarterly Sussex Warren Chronic Disease Coalition meeting in August 2016.

Objective #9e:

An Organizational Chart for the Sussex Warren Chronic Disease Coalition will be reviewed and updated at the end of each quarter for the 2016-2017 grant cycle.

Objective #9f:

Recruit one or more additional chronic disease stakeholders to the Sussex Warren Chronic Disease Coalition membership list during the 2016-2017 grant cycle.
