If you need to probate a Will or seek appointment of Administration, reference the downloadable worksheet form.

This worksheet serves as a general information sheet so that we may prepare the necessary documents for your review and signature. The Assets and Debts worksheets are also completed when there is no Will.

It is as convenient as 1,2,3:

  1. Complete the worksheet
  2. Fax or mail to this office with a copy of the Death Certificate, Will, if there is one, or Assets & Debts, if there is no Will. (You will present the certified Death Certificate and originally signed Will at the time of your appointment.)
  3. Call for an appointment to present the original documents and sign the applications.
