The Sussex County Department of Environmental and Public Health Services, in collaboration with the New Jersey Department of Health, is conducting a Superstorm Sandy recovery survey of Sussex County residents. We are assessing the storm’s impact to residents, focusing on their status in the recovery process. Through this survey we hope to identify what resources were utilized during and after the storm, and what assistance may still be required. No personal identifying information is asked at any point during the survey. Upon completion, participants will be given contact information to request further assistance should they need it.

A printable Superstorm Sandy survey can also be found on this website at http://www.sussex.nj.us/documents/health/superstormsandysurvey.pdf. We encourage you to share this information and post the survey link on your own organization's website. Please feel free to reproduce it and distribute to individuals who may not have access to a computer and were impacted by the storm.

Thank you in advance for your participation in this endeavor. Should you have any questions, please feel free to contact Julia Gurriell of the Sussex County Department of Environmental and Public Health Services by email at BioGurriell@sussex.nj.us or telephone at 973-579-0370, ext 1243.
