PROJECT HEALTHY LIVING COMES TO SUSSEX COUNTY
Are you or a loved one dealing with a chronic condition that poses challenges to your daily activities and enjoyment of everyday life? The Sussex County Division of Senior Services, in collaboration with NORWESCAP RSVP, Newton Memorial Hospital, the NJ Department of Health & Senior Services – Division of Aging and Community Services, and the NJ Department of Human Services - Division of Medical Assistance and Health Services will be offering “Project Healthy Living,” a FREE six-week course based on a chronic disease self-management program developed at Stanford University.
Each week, the interactive class led by a team of Peer Leaders, who are also dealing with chronic conditions themselves or as caregivers, will assist participants in building the skills necessary to manage symptoms; communicate effectively with healthcare professionals; develop healthy lifestyles including good nutrition and exercise; alleviate fear, anger and frustration; make daily tasks easier; and get more out of life. The 2 ½ hour classes are supplemented with a manual, “Living a Healthy Life with Chronic Conditions,” that is provided to each participant at no cost.
We are currently recruiting Peer Leaders to facilitate our first Project Healthy Living classes. Peer Leaders will benefit from the expertise of Master Trainers who have been certified through the Stanford University Chronic Disease Self-Management Program. Peer Leaders are required to attend 4 full days of training and will be provided with a teacher’s manual that outlines coursework and includes teaching tools. There is no cost for Peer Leader training. Peer leaders are expected to facilitate two courses within the year following their training. Peer Leader training will take place at the Sussex County Homestead, 129 Morris Turnpike, Newton, NJ on Tuesdays, January 29, February 5, February 12, and Thursday, February 21, 2008. Training sessions are scheduled from 9:30 am to 3:30 pm each day and include lunch.
