DHSS Commissioner to Attend "Project Healthy Living" Kickoff

DHSS COMMISSIONER TO ATTEND “PROJECT HEALTHY LIVING” KICKOFF
 
 
NJ State Department of Health and Senior Services Commissioner, Heather Howard, will discuss the state-wide Chronic Disease Self-Management Program (CDSMP) initiative and the awarding of a $10,000 grant to the Sussex County Division of Senior Services at its 2009 “Project Healthy Living” Kickoff on Wednesday, February 25, 2009 at the Farmstead Golf and Country Club.
 
 “Project Healthy Living” is an FREE six-week series of workshops based on a chronic disease self-management program developed at Stanford University. The model has been proven to be effective in empowering those dealing with a chronic conditions,  either themselves or as caregivers, to develop the skills necessary to manage symptoms, communicate effectively with healthcare professionals, develop healthy lifestyles including good nutrition and exercise; alleviate fear, anger, and frustration; make daily tasks easier a and get more out of life.  The 2 ½ hour workshops, facilitated by trained volunteer Peer Leaders, are supplemented with a manual, “Living a Healthy Life with Chronic Conditions,” that is provided to each participant at NO COST. Research indicates that individuals who participate in chronic disease self management programs experience fewer emergency room visits, fewer hospital admissions, and shorter hospital stays. 
 
The public is invited to learn more about “Project Healthy Living” by attending the FREE Kickoff event sponsored by the Sussex County Division of Senior Services.     Pre-registration is required. For additional information, contact Sussex County Division of Senior Services at 973-579-0555 or seniorservices@sussex.nj.us.
