Diabetes Self-Management Program

The Sussex County Department of Human Services, Division of Senior Services will be facilitating a FREE six-week Diabetes Self-Management workshop beginning on April 15, 2014, in Newton, New Jersey. Individuals with type 2 diabetes or caregivers of loved ones with diabetes are encouraged to attend the workshop. Each weekly session is 2 ½ hours, and the participants will learn techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as anger and fear. Participants will also learn how to eat healthy, work more effectively with their healthcare providers and how exercise can maintain and improve strength and endurance.

The Diabetes Self-Management Program is based on the Chronic Disease Self-Management Program created at Stanford University's Patient Education Research Center. It was originally developed in Spanish and after receiving successful outcomes with the program, the Stanford Patient Education Research Center received a grant from the California HealthCare Foundation for a controlled study to be conducted for effectiveness on English-speakers, which was completed in 2008. After the study was completed, it was revealed that six months after conclusion of the workshops, participants saw improvements in their symptoms of hypoglycemia, were able to communicate more effectively with their physicians, were eating healthier and saw improvements with depression. Twelve months after completing a workshop, participants continued to see improvements with depression, self-efficacy, healthy eating, patient activation and were still able to effectively communicate with their physicians.

The Diabetes Self-Management workshop will be held on Tuesdays, beginning April 15, 2014 through May 20, 2014, from 1:00 p.m. to 3:30 p.m., at Liberty Towers, 32 Liberty Street, Newton, New Jersey. The program includes a FREE manual, "Living a Healthy Life with Chronic Conditions." Class size is limited and registration is required.
