NEW DIABETES SELF-MANAGEMENT PROGRAM
(Sparta, NJ) The Sussex County Department of Human Services, Division of Senior Services is pleased to announce a new self-management program for those who are dealing with or are at risk for diabetes. Based on the evidence-based Chronic Disease Self-Management Program developed at Stanford University, a series of six goal-oriented workshops, of two and one half hours each, are facilitated by trained volunteer leaders. Participants will learn how to identify and manage symptoms; develop healthy eating and exercise habits; ensure proper foot care; communicate effectively with medical professionals; manage fear, anger, and frustration; relax and think positively; and prevent or delay complications.
The Diabetes Self-Management Program is offered under Title IIID of the Older Americans Act, as amended, at NO CHARGE. The workshops are complemented by a FREE manual, “Living a Healthy Life with Chronic Conditions.” The program will take place at Sparta Health and Wellness Center, on
Tuesdays, May 11, 18, 25, June 1, 8, 15, 2010, from 10 a.m. to 12:30 p.m.
