Help Is Available For Seniors Dealing With Chronic Conditions
Seniors dealing with chronic conditions and/or their caregivers are invited to participate in "Project Healthy Living," a FREE six-week, 2 ½ hour per week, course based on a chronic disease self-management model developed at Stanford University. The program is a collaboration of the Sussex County Division of Senior Services, NORWESCAP RSVP, Newton Memorial Hospital, the NJ Department of Health & Senior Services – Division of Aging and Community Services, and the NJ Department of Human Services - Division of Medical Assistance and Health Services. Each week, the interactive class led by a team of Peer Leaders, who are also dealing with chronic conditions themselves or as caregivers, will assist participants in building the skills necessary to manage symptoms; communicate effectively with healthcare professionals; develop healthy lifestyles including good nutrition and exercise; alleviate fear, anger and frustration; make daily tasks easier; and get more out of life. The course is supplemented with a manual, "Living a Healthy Life with Chronic Conditions," that is provided to each participant at no cost. The next course is scheduled to take place at:
